2/22/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Laylah upon arrival to the home setting. Upon arrival to the home setting, Laylah appeared to be in a jovial mood evident by her facial expressions as well her overall communication with QMHP-C. QMHP-C inquired about her day. Laylah verbalized to QMHP-C that her day was going well. QMHP-C informed Laylah that this session was going to focus on ways to control her anger in a controlled manner at which mother reports behaviors last 20 minutes in duration. 2. Behavior: Laylah actively participated in the session by cooperating in discussions and expressing her feelings appropriately. Laylah reported to QMHP-C on how she was going to learn how to control her anger because she does not want to continue to get in trouble with her mother and when things does not go her way she will isolate herself from her mother and others within the home with behaviors lasting up to 60-90 minutes in duration. 1.Intervention: QMHP-C began session by inquiring about Laylahâs current thoughts, feelings and emotions. QMHP-C verbalized that the session would focus on anger management techniques due to reports from Ms. Boone as Laylah has increased her attitude. QMHP-C modeled calming techniques and relaxation skills to support with stabilizing her moods. QMHP-C assisted Laylah with identifying her triggers and fuses that causes her to become upset and frustrations. 2. Intervention: QMHP-C reiterated journal writing and problem solving techniques. QMHP-C processed on journal entries with Laylah as she struggled with topics that helps to express her thoughts and feelings. QMHP-C engaged Laylah in a therapeutic activity as it relates to conflict resolution and how to handle her anger triggers. Throughout the session, QMHP-C processed on age-appropriate decision-making, along with building effective communication. 1.Response: Laylah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Reports from Ms. Shabazz revealed that Laylah had been displaying a lot of attitude evident by being disrespectful and talking back. Laylah reported that discussed with QMHP-C that she struggles with her anger and that it is difficult to calm down when upset. 2.Response: Laylah verbalized to QMHP-C that she does struggle with identifying her triggers. Laylah reported that she has difficulty with journal writing as she feels that she doesn't know what to write and that she doesnât like writing she just wants to handle the situation when it occurs. During the therapeutic activity, Laylah was able to provide QMHP-C with positive ways to handle conflict as she discussed with QMHP-C that she doesnât utilize those techniques.
